Ontario Health Teams

In the transformation of healthcare currently underway, SAFHT is heavily engaged in defining its role while supporting its physicians and patients. SAFHT is now an active member of two Ontario Health Teams:

  • The East Toronto Health Partners Ontario Health Team; and

  • The Scarborough Ontario Health Team.

The SAFHT physicians are members of the two new physician organizations:

  • The East-Family Practice Network (East-FPN); and

  • The Scarborough Family Practice Network (Scarborough-FPN).

What is Primary Care?

The Ministry of Health defines primary care as "...the patient's first point of contact with a doctor or a healthcare team." Primary care can include disease management and prevention (such as diabetes and other chronic diseases), disease cure, palliative care, health education and promotion and referrals to specialists.

What are Family Health Teams?

The Ministry of Health defines a Family Health Team as "...an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient. Designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurse practitioners, nurses, and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible." Family Health Teams are a key piece of the provincial government's plan to build a health care system that promotes good health, reduces wait times and improve access to care. Family Health Teams focus on preventing illness and promoting health. They will look at the best ways to manage chronic illnesses such as diabetes and depression. Family Health Teams provide care that is more accessible, coordinated, and comprehensive. They will give patients the opportunity to be more involved in making decisions about their own health.
