Programs and Services

In addition to traditional medicine, The Scarborough Academic Family Health Team (SAFHT) coordinates and delivers a broad range of health-care services, with a focus on health promotion, disease prevention and chronic disease management.

As an enrolled patient in SAFHT, the following programs and health services are available to you at no cost. Please speak to your Doctor, or Nurse about a referral to these FREE services.

Chronic Disease Management

Using a team approach, SAFHT will work with you to help manage chronic diseases including cardiovascular disease, high blood pressure, diabetes, kidney disease, weight management, smoking cessation, and breathing disorders. Our goal is to prevent or decrease the incidence of complications associated with chronic diseases through patient education and ongoing monitoring.

In particular, SAFHT offers education and counselling to support at risk patients and those who have Type 1 and Type 2 Diabetes. An initial assessment may be completed by a Nurse Practitioner, Registered Nurse, Diabetic Nurse Educator, or Registered Dietician.

As well as promoting self-care, the SAFHT team develops a management plan for each individual with diabetes to help them understand how to relieve diabetes symptoms, prevent or treat complications, and improve the quality of life. The goals of education and counselling are to:

  • reduce acute diabetes complications

  • decrease foot problems

  • shorten or eliminate the need for hospitalization

  • improve long-term blood sugar control

  • reduce and/or prevent the onset of long-term complications, and

  • support self-management.

Chiropody (Foot Assessment)

Foot problems are common in people with Diabetes and other chronic conditions and can lead to serious complications. The SAFHT Foot Care Clinic aims to provide patient education and individualised foot care services to prevent foot problems and potential serious complications, teach foot care self-management including reducing foot injury, proper foot wear or self inspection and recommend follow up with appropriate healthcare professional where indicated. Mental Health

Our confidential services are free of charge and include:

Assessment

Upon referral to the mental health program, patients will undergo a comprehensive assessment. Patients will have the opportunity to work collaboratively with a mental health clinician to set and achieve individually defined treatment goals. Depending on the needs of the patient, a referral to community services may be facilitated.

Psychotherapy

Short term counselling is offered for up to 8 – 12 sessions with a member of our mental health team. The mental health team uses a variety of treatment modalities including Cognitive Behavioural Therapy (CBT), Relational Psychotherapy, Interpersonal Psychotherapy (IP) and Mindfulness Based Stress Reduction (MBSR) to aid patients in the process of understanding, managing and where possible, resolving their mental health difficulties. Upon completion of their sessions, patients may be referred to more specialized mental health services for longer term support.

Resource Counselling

SAFHT's mental health clinicians can provide patients with information about available community resources and facilitate access to these resources.

System Navigation

SAFHT's mental health clinicians work with patients to coordinate transition points and movement through complex health care and social service systems.

Case management

SAFHT's mental health clinicians work collaboratively with other service providers to facilitate overall coordination and continuity of a patient's treatment plan.

Advocacy

SAFHT's mental health clinicians advocate on our patient's behalf within the healthcare and social service sectors to facilitate access to resources such as housing, income support, home care etc.

Group Support

SAFHT offers a range of groups and workshops that respond to a broad spectrum of mental health issues including: depression, anxiety, stress, trauma, healthy relationships etc.

Mental Health Promotion

The mental health team is involved in a number of health promotion efforts to raise awareness about issues related to mental health. The team produces a quarterly newsletter called Changing Minds that educates patients about a wide range of mental health issues.

Stop For FHT (A stop-smoking program that is under development)

This program is designed for smokers who want to quit. NRT (Nicotine Replacement Therapy) and counselling will be provided to individual SAFHT clients by health care professionals who have trained through TEACH (Training Enhancement in Applied Cessation Counselling and Health). The Program is offered by the Centre for Addiction and Mental Health and the Ministry of Health Promotion. Studies show that combining NRT with counselling doubles the chance of successfully quitting.
