Case Management Services

Case management facilitates connections to clients needs and resources through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the needs and values of the client, and in collaboration with internal and external service providers. The case manager links clients with appropriate providers and resources throughout the continuum of health and human services and care settings, while ensuring that the care provided is safe, effective, client-centered, timely, efficient, and equitable. The case manger also provides individual counselling, brief consultation, group facilitation, internal case assignment to our mental health team and external resources. Does not provide housing services, will link to an appropriate service provider, if needed.
