FOCUS Program

The FOCUS program provides home-based primary care for clients unable to physically come into the office due to disease and disability. The care is comprehensive and continues right through to your final days.   

This program requires a referral from your family doctor.

Once accepted the Nurse Practitioners will work collaboratively, with a variety of people in your circle of care, to meet your agreed upon goals of care. In this arrangement the Nurse Practitioner becomes your main care provider, and your family doctor remains available when necessary. 

The Nurse Practitioner will provide holistic health care through:  

  • Seamless management of your medical conditions 

  • Regular and urgent medical visits to prevent unnecessary hospitalizations 

  • Connecting you with services in the community to assist with activities of daily living 

  • Review of your environment for safety concerns so you can carry on with confidence 

  • Advanced care planning to make sure those around you understand your values and wishes in the event you’re unable to speak for yourself  

All of this is done in collaboration and with respect for your wishes and goals of care.
