Overview of Diabetes Conversation Map

The four Conversation Map education tools are: 

The Living with Diabetes Conversation Map experience is an overview of Diabetes, including what diabetes is, and some of the most common myths about diabetes. The experience gives the participants an opportunity to discuss their feelings about diabetes. Participants also learn more about good control which includes general targets for blood glucose levels, A1c, blood fat levels, blood pressure, and waist circumference. Finally, participants have a brief discussion about the things they can do to manage their diabetes. 

The How Diabetes Works Conversation Map experience involves a more in-depth conversation about the Pathophysiology of diabetes. It specifically explores what is happening inside the body of a person with Diabetes. It also explores what happens if diabetes is not under control and some of the complications that can result. 

The Healthy Eating and Keeping Active Conversation Map experience focuses on, the lifestyle changes people with Diabetes can make, to have a better quality of life and avoid or delay complications related to poor control of diabetes. The experience gives the participants the opportunity to identify any challenges they might face towards making changes and how to overcome those challenges. 

The Starting Insulin Conversation Map experience covers the basics of starting insulin treatment, including why insulin treatment will be necessary for many people with Diabetes. Participants learn about the different types of insulin, how they work, injection sites and rotation rules, They also discuss hypoglycemia: what causes hypoglycemia, and how to treat and avoid it.

 Finally, participants learn how to identify blood glucose patterns and make adjustments to address these patterns.

Diabetes Program

Individual Diabetes Education

Registered Nurses provide diabetes education for newly diagnosed patients. During these sessions they provide information about diabetes, provide glucometers and instructions on how to use them. 

Patients can receive ongoing health teaching sessions with a nurse for their bloodwork, blood pressure, blood sugar, foot check , weight, height and body mass index.

They will also receive reminders about eye checks, and medications reviews. The nurse will communicate with the patient’s doctor and provide some recommendations if needed. They will also work with the doctor if the patient needs a referral to an endocrinologist for uncontrolled diabetes. 

Insulin and Ozempic start, phone calls for adjustments of medications, and working with patients to make sure they are pre-booked for a 3 months follow up are also services provided. 

Referrals to  a dietitian, chiropodist and/or mental health worker will also be provided as needed.
