In order to access our individual counselling or case management services a referral from a doctor is required. If your doctor is a part of the Scarborough Academic Family Healthy Team, please ask them for a referral. If they are not, then please have them go to our referrals page and fill out the proper form and fax it to our office. The fax number is on the referral form.

Our mental health team sees patients by phone, video or in person at the following locations:

1. 523 Victoria Park Avenue (Victoria Park and Danforth) 

2. Suite 320, 2330 Kennedy Road (Kennedy and Sheppard) 

3. Unit 5, 1330 Kennedy Road (Kennedy and Ellesmere)

Groups are open to the public. If you would like to register for a group please go to our registration page and select the group(s) and/or workshop(s) that you are interested in. Someone from our office will be in touch, to give you further information.
